Preparation of Ferric Citrate.

1, citric acid is added to the iron hydroxide solution, heated to below 60 ℃, evaporated and concentrated to slurry, and obtained by low-temperature drying on a glass plate. 

2, add ammonia to iron sulfate solution to produce iron hydroxide precipitation. Filter and wash with water until there is no reaction of sulphuric acid. Add citric acid and heat to below 60℃ with stirring to dissolve all the precipitate, then filter. Concentrate the filtrate into syrupy form by heating below 60℃, coat it into a thin layer on a glass plate, form small leaves, peel it off, slightly warm and dry it.

Mechanism of Action

The therapeutic effects of ferric citrate stem from its unique mechanism of action. After entering the body, ferric iron (Fe³⁺) is absorbed in the gastrointestinal tract via metallotransferase-1, a process that requires reduction to ferrous iron (Fe²⁺) by ferric reductase and cytochrome B reductase.

The absorbed iron undergoes several fates in the body: some is stored by ferritin; some enters the bloodstream via ferrotransferases and is ultimately carried to tissues that require iron.

Regarding phosphorus binding, the iron ions in ferric citrate bind with phosphate in the gastrointestinal tract, forming insoluble ferric phosphate, which is excreted in feces, thereby reducing blood phosphorus concentration.

This mechanism is particularly important for patients with renal insufficiency who cannot excrete phosphorus normally, effectively preventing complications caused by hyperphosphatemia.

It is worth noting that the bioavailability of ferric citrate is approximately 19% of that of ferrous ascorbate, meaning its iron absorption efficiency is relatively low, but this also reduces the risk of side effects from excessive iron intake.
