5-Methyltetrahydrofolate is the most biologically active and functional form of folic acid, the only way for folic acid to participate in physiological metabolism, and the only form of folic acid that appears to be present in animal plasma and cellular free folate.

It is mainly used as an active ingredient in pharmaceuticals and as a food additive, with applications in the prevention of fetal neural tube defects, atherosclerosis, the treatment of megaloblastic anemia, the enhancement of the therapeutic effect of pyrimidines fluoride, and the treatment of autoimmune diseases such as psoriasis and rheumatoid arthritis.

Widespread research in China has shown that there is a higher percentage of mutation defects in the MTHFR (a key gene for the conversion of active folic acid in vivo) gene in China than in Europe and the United States, resulting in a failure to efficiently convert active folic acid metabolism into action and causing cellular dysfunction.
